 Kansas City Life offers two Non-Contributory plans.  They are available to groups of 2 or more employees with 100% participation required (no waivers):

• For information on the $10-$25 Co-Pay Plan .
• For information on the $0 Co-Pay Plan .

• For information on the $10-$25 Co-Pay Plan .
• For information on the $0 Co-Pay Plan

Davis Vision network of providers has more than 30,000 service providers.  To locate a Preferred Providers and clicking on the "Members" site and entering the Client Code: 2834 in the Open Enrollment/Discount Plan seciton of the Member Site.

Agent commissions on these plans are a flat 10%.
