This product is available for online enrollment only!

To obtain the link necessary to make this product available to your clients you will need to complete the following steps:

1. Download contracting and appointment forms.  Complete them, attach a copy of your license and E&O Coverage and send them to us via the address provided on the forms, or fax them to us at 913-945-4393 or scan and email them to us at sales@allied-brokerage.com.
2. Once you have been appointed we will provide you with a link with a unique id that identifies you as the agent on any application your clients complete. 
3. You will receive commissions on all policies issued as a result of your clients using this link to apply for the Vision for Everyone product.
