In addition, for takeover groups with at least 10 enrolled employees, all waiting periods are waived for current and new hires.

Group CONFIDENT UHIC Dental Plan

Basic Plan (In Network): 100/90/60, 80% R&C, $50 Annual Deductible, $1000 Maximum*

The following options may be available based on group size and contribution:

• $1250, 1500 or $2000 annual minimum
• Elimination of waiting periods
• Orthodontia $1000 or $1500 lifetime maximum
• Endodontics, Periodontics and Oral Surgery in basic
• 90% Reasonable and Customary
• $25 Deductible
