cbg's CONFIDENT Dental is backed by the rock-solid underwriting of Transamerica Life Insurance Company.  There are employer paid plans down to two lives and voluntary plans down to 5 participating lives or 25% of all eligible employees.

Group CONFIDENT UHIC Dental Plan

Basic Plan (In Network): 100/90/60, 80% R&C, $50 Annual Deductible, $1000 Maximum*

The following options may be available based on group size and contribution:

  • $1250, 1500 or $2000 annual minimum
  • Elimination of waiting periods
  • Orthodontia $1000 or $1500 lifetime maximum
  • Endodontics, Periodontics and Oral Surgery in basic
  • 90% Reasonable and Customary
  • $25 Deductible

* The CONFIDENT Incentive PPO plan is not available in all states. We offer a passive PPO or an indemnity plan in those states where the Incentive PPO is not available.

Basic filings; Space maintainers; Sealants (children to age 16); Simple Extractions

  10% Usual, Reasonable, and Customary Benefits

50% after One Year

TYPE III – MAJOR SERVICES

Oral Surgery; Endodontic Treatment; Periodontic services; Crowns, inlays and onlays; Prosthetic services, bridges and dentures; One diagnostic
x-ray, full or panoramic in any 3 year period.
